| The Health Literacy and Community Education Program at The Greater Hudson Valley Family Health Center (GHVFHC) is dedicated to empowering patients and community members to increase control over, and to improve physical, social and mental health by providing accurate, and accessible information about health and its determinants. The Health Literacy and Community Education program further endeavors to collaborate with other health and human service organizations to foster a sense of shared community and to best provide the diverse learning opportunities that support the inherent ability of all individuals to create health and wellness for themselves, their families, and society. Health Literacy and Community Education CenterGHVFHC supports a broad array of health and human service programs to better empower patients and the members of our community to take positive action towards improving personal health and well-being. In recognition of the strong impact that social determinants have on health outcomes, the Health Literacy and Community Education Center was recently constructed to house diverse human service programming as well as health education programs, in a safe, inviting and dignified environment. The programs are the most extensive and comprehensive health education programs in the region. We are committed to ongoing development of offering effective programs and will continue to regularly partner with other health and human service organizations to bring a broad range of workshops, support groups and classes to meet the diverse needs of our community.
