Care Management Director

We are currently looking for a Care Management Director to run the Care Management Department in our Newburgh, NY facility. The Director will coordinate the care of patients by making sure they receive timely, comprehensive healthcare services.  Duties include administrative oversight by engaging with participating agencies across the Hudson Valley region, monitoring state and federal policy as related to care management, and attending regional health home meetings.

The Director will be responsible for supervising the newly expanded Care Team as well as maintaining and troubleshooting the integration with clinical departments. Chemical Dependency Care Coordinator

We are currently looking for a Chemical Dependency Care Coordinator in our Care Management Department at our main site in Newburgh, NY. The Care Coordinator will provide care coordination and integration of services to GHVFHC client population at our Center for Recovery (CFR). CFR offers chemical dependency treatment services to assist individuals into recovery.

Responsibilities include: providing care coordination activities for clients receiving services at CFR to ensure linkage, engagement, treatment adherence, and monitoring of primary care services. As well as screening patients for substance use and providing evidence based behavioral change modalities (motivational interviewing, stages of change, etc.) to engage patient in recovery.

All applicants should have: Credentialed Alcoholism and Substance Abuse Counselor (CASAC) credentials or equivalent. Applicants need to be highly organized with excellent oral and written communication skills, as well as the ability to maintain a non-judgement disposition and communicate with a diverse population. Part-time Breast Feeding Peer Counselor

We are currently searching for two Part-time Breastfeeding Peer Counselors to aide our very busy Women Infant Children (WIC) Department at our Health Center in Newburgh, NY. Each position is part-time working around 15 hours per week. Flexibility with schedules is a must!

Candidates are responsible for providing counseling and support throughout the patient's pregnancy as well as while breastfeeding. Candidates also will utilize counseling skills acquired during periodic mandatory trainings. We are searching for a Full-time and Part-time Bilingual Patient Services Representative to aide our busy Dental Department. Applicants will be responsible for general office duties including heavy phone volume, patient registrations, collecting of co-payments, and verifying insurance information. The Representative also should be responsible for monitoring the patient waiting area and ensuring that it is kept clean and orderly.

Housekeeper

We are currently in search of a Housekeeper to ensure that all GHVFHC facilities are clean and safe, for its patients, staff and guests. Housekeeping responsibilities include cleaning and disinfecting all surfaces such as floors, counters, exam tables, chair and bathrooms. Other duties include removing trash and disposing of "red bag" materials appropriately as well as restocking supplies in each department, as assigned by the Maintenance Supervisor.

The Housekeeper will be held accountable for adhering to GHVFHC Policies and Procedures, OSHA Regulations and Department of Health guidelines regarding cleaning, sanitization and safety in medical facilities.

We are looking for a Part-time Retail Assistant to assist in our retail/consignment store (ENCORE) located in Cornwall, NY. The assistant will aide in the development and maintenance of inventory, and develop Brand charts.
