First Time Patients

Welcome to our practice and thank you for choosing The Greater Hudson Valley Family Health Center (GHVFHC) to be your health care provider. We understand that your time is valuable, so in order to expedite your visit, below is a list of items you will be required to provide upon your arrival to the health center.

Please bring the following items to your first visit:

• Picture identification i.e., passport, driver’s license, green card (For children, please bring their birth certificate)
• Insurance card(s)
• Co-payments (We accept most major credit cards and checks for payment)

Most non-insured patients with appropriate documentation can be enrolled in our Sliding Fee Scale Program (see sliding fee scale section below) and will be eligible to pay a lower fee per visit.

Income verification will be required for all non-insured patients. If income verification is not provided, please be prepared to pay $149.00 dollars for the visit.

If your health care provider orders lab work, medical treatments, immunizations, PPD’s, EKG, etc., an additional cost will be incurred and you will be required to pay at the time these services are rendered to you.

Sliding Fee Scale

Sliding Fee Scale Qualifications:

Those who are interested in applying for our Sliding Fee Scale Program should speak with a Patient Services Representative at (845) 563-8016. We recommend that you allow at least one hour prior to your appointment to apply for these services.

Documentation of household income for yourself and any individual residing with you is require when applying for sliding fee services. Any of the following documentation is acceptable:

• Last four pay stubs
• W2 tax statements
• Proof of disability income
• Child support statements and/or a letter of support and unemployment benefit letter

Our Patient Services Representatives can assist you with qualifying your child for health insurance if he/she is uninsured and/or undocumented.

Also, multiple managed care plans have representatives on-site who you may speak with about low or no-cost health insurance.

Enabling Services

GHVFHC's Enabling Service Department provides various services for our patients in need. The following services include:

• Sliding scale fees
• Medicaid/Pre-natal Medicaid
• Food stamps through Supplemental Nutrition Assistance Program (SNAP)
• Home Energy Assistance Program (HEAP)

All of the above services provided by our Enabling Service Department are all subject to eligibility. GHVFHC also works with the following community organizations to assist patients who may need additional services that we do not provide:

• Maternal Infant Services-Network (MISN)
• AHRC
• Safe Homes of Orange County
• Jewish Family Services  of Orange County
• Salvation Army
• People for People Fund

GHVFHC's Outreach Program also is utilized by our clinical staff in an effort to always be able to contact our patient about their health status. For any given situation, GHVFHC outreach workers make home visits in the event that we are unable to reach patients by telephone, mail, or any other designated communication to inform them of concerns/risks regarding their health.

At GHVFHC our mission is to provide comprehensive high quality health care to all who require our services, regardless of their ability to pay.

However, because of the programs offered by the federal and state government and many insurance carriers, it is easier than ever to be enrolled in a health care plan for you and your family based upon qualifying factors such as income and family status.

Participating insurance carriers include:

There are many locally-based insurance programs, most of which are provided by unions or small employers that cover some of our services. If you are a participating member of one of these carriers please be sure to check with the carrier to ensure that your carrier will cover the services provided by GHVFHC.
